Application Instructions

Applicants for active membership must have completed their training three years prior to election to membership. This does not apply to Vascular Fellows completing their Vascular Fellowship from an "approved" Vascular Fellowship Program. These individuals may apply for Active membership upon completion of the program.
Make sure all questions are answered completely and that two individuals are listed as references. It is the responsibility of the applicant to be certain that letters of reference are transmitted to: Eastern Vascular Society, 19 North Street, Salem, MA, 01970. It is preferable if at least one person listed as a reference is not in partnership practice with the applicant, nor a member of the same academic department.
Completed applications must be received by May 15, 2008.
